Families of children and young people aged 0–25 with Special Educational Needs and Disabilities (SEND) are invited to take part in the Leeds Parent Carer Forum Annual Survey 2026.
The Leeds Parent Carer Forum (PCF) work with Leeds City Council, schools, health services and social care to ensure families’ voices shape services. Your experiences will help improve support across Leeds.

Your responses help improve services, share experiences with decision-makers, and track change

Your responses help highlight what is working well, where improvements are needed, and ensure families’ experiences are shared with decision-makers
The survey covers education, health, social care, EHCPs, communication, and preparation for adulthood. This year’s survey includes questions on neurodevelopmental support, mental health, and hospital care.

Prize draw:
As a thank you, two parent carers who complete the survey will be selected at random to receive a £30 Amazon voucher. Entering the prize draw is optional.
