Family Information Survey - Please help us to get our records up to date

We've decided during this time we will take the opportunity to make sure we have up to date and correct details for as many of our Sunshine & Smiles families as possible. We also want to make sure we have up to date consent for sending emails, letters & messages.
We would be hugely grateful if everyone who is part of Sunshine & Smiles could take 5 - 10 minutes to complete our Family Information Survey - click here.
We will also be sending out a further survey in the near future to ask people's views on how they feel about how and when we should consider re-starting our physical group activities once government guidance permits.
