Emotional and Mental Wellbeing Support for Children and Young People
Leeds Community Healthcare are looking at services in Leeds in relation to children and young people aged 0-18yrs who need emotional and mental wellbeing support.
The project will help families, carers and others (e.g, GP’s) to really understand what is available across Leeds, and with the input from parents/carers, will produce a booklet that is helpful and understandable and that gives information on these services. This booklet will be available in paper form and online.
The deadline to complete the survey is the 20th May 2022.
If you would like to complete a hard copy of the questionnaire, please download here and return to Donnaryan2@nhs.net by Friday 20th May 2022
